According to a recent study published in the Journal of the American Academy of Dermatology, patients treated with IL-23 inhibitors are at a lower risk of developing inflammatory arthritis compared to those receiving IL-17 and TNF inhibitors.
The relationship between biologic treatments for psoriasis and the development of inflammatory arthritis in patients remains unclear. This study aimed to investigate how these biologic treatments influence the risk of developing inflammatory arthritis in individuals with psoriasis.
Researchers conducted an analysis of patients with psoriasis from a large database, focusing on those who did not have a prior diagnosis of inflammatory arthritis. Patients were categorized based on the type of biologic treatment received (IL-23, IL-12/23, IL-17, or TNF inhibitors) and were monitored for up to three years or until the onset of inflammatory arthritis. The risk of developing arthritis was assessed using a multivariate statistical model, with IL-23 inhibitors serving as the reference group.
The incidence rates of developing inflammatory arthritis varied among treatments. IL-23 inhibitors demonstrated the lowest rate (4.99 events per 100 person-years), followed by IL-17 (7.29), IL-12/23 (6.06), and TNF inhibitors (9.39). Patients receiving IL-17 and TNF inhibitors exhibited significantly higher risks of developing arthritis compared to those on IL-23 inhibitors.
Researchers say that further studies are needed to ascertain whether IL-23 inhibitors actually reduce the risk of developing inflammatory arthritis for people with psoriasis.
The study acknowledged limitations, including potential medical coding errors and biases related to the timing of treatment. In addition, the study did not include patients taking Disease-Modifying Antirheumatic Drugs (DMARDS), nor did it include those not taking biologics.
